My future, my choice
My future, my choice is helping students with significant disabilities and high, complex support needs to make informed choices about accessing services and activities beyond school. Now in its sixth year, the program creates smoother transitions for students through collaboration between state and non-state schools, post school service providers and disability and community care services. Each year, about 21 students from five schools sample a range of programs offered by nine services, with more than 78 per cent of stakeholders crediting the placements for helping students make post school service provider and activity choices the following year. The program’s success, and the overarching transition planning model behind it, have been recognised in a university textbook as best practice.
