Planning Alternative Tomorrows with Hope (PATH)
The PATH is a graphic facilitation to develop an achievable and realistic goal for the future based on 'the dream' and implements backwards planning to create a step-by-step path to achieving that goal.
Personal Future Action Plan (PFAP)
The PFAP is reviewed each semester, and encompasses short and long-term goals across the areas of Vocational, Living Arrangements and Social and Recreation. Strategies are developed to support students to achieve their goals, and different environments are considered- including home, school and partnerships.
